Your Role:

• Provide compassionate care with dignity and respect for residents
• Monitor vital signs and report changes in resident behavior to the nursing and medical team
• Assist residents with daily care needs while maintaining comfort and safety
• Routinely make rounds and provide responsive resident care and services
• Support resident independence and well-being through patient-centered care
• Maintain accurate communication with nursing staff regarding resident needs
• Ensure reliable attendance and punctuality for assigned shifts
• Follow facility policies, procedures, and long-term care standards
• Perform additional duties as assigned by management in a resident-centered care environment

Who We're Looking For:

• Current Illinois CNA certification required
• Graduate of an accredited CNA program
• Caring, dependable, and team-oriented attitude
• Passionate about providing personalized care to residents
• Strong communication and teamwork skills
• Dependable with reliable attendance and professionalism
• Motivated to grow professionally within long-term care
