Community Integrated Case Manager Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. Job Summary The Community Integrated Case Manager provides case management services in shelter or housing programs and serves as the liaison between the CBI Shelter and Housing Programs and the CBI PCMH clinic. The role assists patients and treatment teams in planning, connecting, outreaching, advocating, coordinating, and monitoring patient needs and progress while in the program. It also helps patients gain access to health services, financial assistance, employment, education, social services, and natural supports, and develops independent living skills. The Case Manager assesses patient needs, develops and monitors individualized treatment plans, and documents services accurately and timely. Employees must embrace the recovery philosophy and promote a safe recovery environment for clients. Skills / Requirements High school diploma or GED and either: 18 credit hours of post‑high‑school education in a field related to behavioral health and two years of full‑time behavioral health work experience; or four years of full‑time behavioral health work experience. Associate’s degree and at least two years of full‑time behavioral health work experience. Bachelor’s degree in a field other than behavioral health and at least one year of full‑time behavioral health work experience. Master’s degree or bachelor’s degree in a field related to behavioral health. 1–3 years of experience in a medical setting (preferred). Minimum of six months of recovery from substance use and/or mental health disorders (preferred). Current AZ Driver's License (valid and in good standing). Behavioral Health Technician (BHT) certification in accordance with A.A.C. R9-10-101 and CBI Clinical Policies and Procedures. Clear 39-month motor vehicle record.
