You will collaborate with physicians, families, and interdisciplinary team members to provide high-quality, patient-centered care.Key ResponsibilitiesPerform patient evaluations and develop individualized treatment plansProvide therapy to improve strength, balance, mobility, and overall functionMonitor patient progress and adjust treatment plans as neededMaintain accurate and compliant clinical documentationCommunicate with physicians, residents, families, and care team membersParticipate in care planning, rehab meetings, and discharge planningConduct staff education and in-service trainingSupervise PTAs, aides, and studentsQualificationsDegree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)Active, unencumbered Physical Therapist license in South Carolina (required)Must maintain all continuing education and remain in good standing with the State BoardExperience in a skilled nursing or rehab setting preferredFamiliarity with PointClickCare (PCC) and Casamba preferredStrong clinical, leadership, and communication skillsWhy Join Fleetwood Post Acute?At Fleetwood, you’ll be part of a stable, experienced team that values your expertise and supports your growth.
