Job Summary

Seeking a compassionate and dedicated Home Health Registered Nurse (RN) for a 13-week travel assignment. This position provides comprehensive nursing care to patients of all ages in their homes throughout Caledonia and southern Essex counties.

The ideal candidate is patient-focused, highly organized, and passionate about delivering quality care while collaborating with an interdisciplinary home health team.

Responsibilities
Conduct patient admissions, assessments, and ongoing evaluations
Develop and implement individualized plans of care
Provide skilled nursing services in the home setting
Monitor patient progress and adjust care plans as needed
Educate patients and families regarding treatments, medications, and disease management
Collaborate with physicians, therapists, aides, and other healthcare professionals
Maintain accurate and timely clinical documentation
Support coordinated care and positive patient outcomes
Required Qualifications
Active Vermont Registered Nurse (RN) License
Minimum 4 Year of Clinical Nursing Experience
Current BLS Certification
Strong assessment and patient education skills
Ability to work independently in a home health environment
Preferred Qualifications
Previous Home Health Experience
Experience with Homecare Homebase (HCHB)
Strong communication and care coordination skills
