Registered Nurse

CHC is looking for a dedicated Registered Nurse to join our Family Practice team at our brand-new Muscatine clinic! In this position, you will learn a variety of skills while providing comprehensive, patient-centered care to individuals and families in our community.

Salary: $31$40/hour

In this key role, you will:

Health Coach Develop care plans and partner with patients to achieve their health goals
Patient Advocate Serve as a liaison between patients, families, providers, hospitals, and community agencies
Connector Identify and link patients to resources that support access to care
Social Determinants of Health Recognize and address psychosocial, cultural, ethnic, and spiritual needs of patients and families
