You will join a supportive home care team that values high-quality outcomes, professional growth, and a sustainable work-life balance. · Flexible scheduling with full-time, part-time, and pay-per-visit opportunities · Meaningful one-on-one patient care in the home setting · Paid time off and comprehensive health insurance packages · Benefits eligibility for colleagues working 24 or more hours per week · Employee assistance and wellness programs with a strong focus on mental health · Company-provided cellphone and laptop for therapists · Discounts on services, products, travel, movie tickets, pet insurance, and optional coverages · Leadership and career-development opportunities at multiple levelsWhat You'll Do · Evaluate homebound patients referred for physical therapy and identify functional needs. · Develop and implement individualized treatment plans based on each patient's goals and condition. · Provide therapeutic exercise and other appropriate interventions to improve strength, mobility, balance, endurance, coordination, gait, and daily function. · Assess pain, joint mobility, motor control, posture, cardiopulmonary function, soft-tissue integrity, and the ability to complete activities of daily living. · Educate patients and caregivers on treatment plans, safe mobility, home exercise, and strategies that support independence. · Collaborate with the care team to coordinate services, monitor progress, and adjust the plan of care as needed. · Complete timely, accurate documentation in accordance with clinical and organizational standards.Qualifications · PT experience required. · Graduate of Physical Therapy Program from an accredited American Physical Therapy Association school, BS degree or entry level master's degree. · Active CT PT license. · CPR certification required at time of start.Regular Standard Hours Per Week : 40 Schedule : Full-time (40 hours) Shift Details : Mon thru Friday Float position for greater Torrington area. Requires 1 weekend day per month and 1 major or minor holiday per year coverage. Will consider less than 40hrs per week position
