Location: Fort Worth, TX | Skilled Nursing Facility
Schedule / Job Type: Contract | Days | 13-Week Assignment
Key Responsibilities:
- Evaluate and treat patients in a skilled nursing facility setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established goals
- Collaborate with interdisciplinary care team to ensure optimal patient outcomes
Required Qualifications:
- Active Physical Therapist (PT) license in the state of Texas
- Graduate of an accredited Physical Therapy program
- Ability to work day shift in a skilled nursing facility environment
Preferred Experience / Skills:
- Prior experience working in a skilled nursing facility (SNF) setting
- Experience with geriatric patient populations
