Registered Nurse Case Manager

Acute Care Case ManagerThis is a 13-week contract position.Day Shift: 5x8-Hour (08:00 - 16:30)The Case Manager will be responsible for managing a caseload of up to 25 patients. This role involves care coordination, discharge planning, and collaborating with interdisciplinary teams to ensure effective patient management. The Case Manager will assess patients, determine medical necessity, and participate in family meetings while actively engaging in the development of treatment plans.Manage a caseload of patients with varying medical and social needs.Coordinate care services within an acute care setting.Facilitate patient assessments and determine medical necessity according to evidence-based guidelines.Plan and implement discharge plans, ensuring smooth transitions for patients.Participate in treatment planning and family meetings.Utilize InterQual criteria for patient assessments and care coordination.Minimum of 2 years of experience in acute care case management or discharge planning.Must hold a valid CA state license by the start date; pending licenses are accepted but must be licensed upon starting.Experience with Epic charting system is required.Strong knowledge of CMS guidelines and Medicare/MediCal regulations is preferred.Ability to independently arrange DME, SNF, and transport services.At ARMStaffing, we take care of our employees!
