Director of Nursing (DON/RN)

Woodsfield Health Care Center is NOW UNDER NEW MANAGEMENT , and we are looking for an experienced, driven, and resident-focused Director of Nursing (DON/RN) to help lead the next chapter of our clinical team.

As Director of Nursing, you will provide overall leadership and direction for nursing services while partnering closely with the Administrator and interdisciplinary team.

Responsibilities include:

• Lead and oversee all nursing operations within the facility.

• Recruit, develop, mentor, and retain a strong nursing leadership and frontline clinical team.

• Establish clear expectations and promote accountability throughout the nursing department.

• Ensure residents receive high-quality, compassionate, and clinically appropriate care.

• Maintain compliance with federal, state, CMS, and facility requirements.

• Prepare the nursing department for surveys and assist with plans of correction when necessary.

• Monitor clinical outcomes, quality measures, incidents, hospitalizations, infections, wounds, and other key indicators.

• Identify areas of clinical risk and implement effective corrective action plans.

• Oversee resident assessments, care planning, documentation, and changes in condition.

• Partner with the Administrator to evaluate staffing levels, scheduling practices, overtime, and agency utilization.

• Develop and mentor ADONs, Unit Managers, RNs, LPNs, and CNAs.

• Promote effective communication between nursing, physicians, families, pharmacy, therapy, and other departments.

• Participate in QAPI, risk management, infection prevention, and other clinical initiatives.

• Create a nursing culture centered around accountability, teamwork, consistency, and resident-centered care.

• Serve as a visible, hands-on leader who is willing to support the team and lead from the front.

• Current Registered Nurse (RN) license in the State of Ohio required.

• Previous Director of Nursing, Assistant Director of Nursing, Unit Manager, or comparable nursing leadership experience strongly preferred.

• Skilled nursing/long-term care experience strongly preferred.

• Strong knowledge of CMS regulations, survey processes, clinical documentation, and quality improvement.

• Demonstrated ability to lead, coach, and develop nursing teams.

• Strong organizational, communication, and problem-solving skills.

• Ability to identify challenges, develop solutions, and follow through on results.

• A leadership style that balances high expectations with genuine support for the team.
