The LPN supports individuals with developmental and/or physical disabilities by ensuring safe medication administration, monitoring health conditions, and collaborating with interdisciplinary teams to promote optimal health and quality of life.This role requires strong clinical judgment, effective communication, and the ability to support and educate direct care staff in accordance with DDS and Medication Administration Program (MAP) guidelines.Key ResponsibilitiesClinical Care and MonitoringProvide direct nursing care in accordance with individualized care plansMonitor and assess residents’ health status; identify changes and report concerns promptlyPerform routine nursing assessments (skin integrity, vital signs, pain, etc.)Assist with medical appointments and follow-up care coordinationMedication Administration and MAP ComplianceAdminister medications and treatments per physician orders and MAP regulationsEnsure proper medication storage, documentation, and chain of custodyConduct medication counts, especially for controlled substancesReport and document medication occurrences per policyCare CoordinationCommunicate with physicians, pharmacies, VNA, therapists (PT/OT/Speech), and familiesAssist in implementing and updating care plansCoordinate lab work, diagnostic testing, and follow-up appointmentsStaff Support and TrainingProvide guidance and oversight to direct support staff on health-related needsReinforce proper techniques (e.g., feeding protocols, positioning, use of adaptive equipment)Participate in staff training related to health and safety proceduresDocumentation and ComplianceMaintain accurate and timely documentation in accordance with agency, DDS, and DPH standardsComplete incident reports and nursing notes as requiredEnsure compliance with infection control and safety protocolsHealth and SafetyMonitor for signs of illness, injury, or decline in conditionRespond to medical emergencies and initiate appropriate interventionsPromote preventative care (hydration, nutrition, skin care, mobility support)Required QualificationsActive LPN license in the state of MassachusettsCurrent CPR/BLS certificationStrong assessment, communication, and organizational skillsPreferred QualificationsExperience in a group home or community-based settingExperience with individuals with developmental disabilities (preferred)Knowledge of DDS regulations and MAP policies (strongly preferred)Experience with electronic medication administration systems (e.g., Impruvon)Physical RequirementsAbility to assist with transfers and mobility (may include Hoyer lifts)Ability to stand, walk, bend, and lift as neededAbility to respond quickly in emergency situationsWork EnvironmentResidential/group home settingMay include evenings, weekends, and holidays depending on scheduleSalary: $36.00 to $39.00 per hour to commensurate with education and experience.Benefits: BCL offers a comprehensive benefits package including medical, dental, tuition, and 401k and life insurance options for all eligible employees.Employment decisions will be based on the principles of equal opportunity.
