The Acute Access Nurse Practitioner, Virtual Care (VSP/PT) works on problems of diverse scope and complexity ranging from moderate to substantial. Nurse Practitioner, Virtual Care (VSP/PT) provides health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions. You will focus on Quality Measures as well as some assistance with acute access. The Acute Access Nurse Practitioner, Virtual Care (VSP/PT) works in our team-based care environment.The Acute Access Nurse Practitioner, Virtual Care (VSP/PT) will provide care for patients through virtual visits. Patient Care: The Acute Access Nurse Practitioner, Virtual Care (VSP/PT) will dedicate their clinical efforts to improving access for the patients we serve. Evaluates and treats center patients in accordance with standards of care. Follows level of medical care and quality for patients and monitors care using available data and chart reviews. Acts as an active participant and lead with the care team through daily huddles. Helps Associate Medical Director (AMD), Physician and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor. Completes all medical record documentation in a timely manner working with a quality- based coder to optimize coding specificity. Follows policy and protocol defined by Clinical Leadership. Meets with AMD about quality of care, review of outcome data, policy, procedure, and records issues. Assures personal compliance with licensing, certification, and accrediting bodies. a Bachelor's degree

• 2 or more years of project leadership experience
• Active, unrestricted Nurse Practitioner/PA license in the appropriate state
• Masters in Science of Nursing /PA

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
