Community-Based NP/PA: Urgent & Transitional Care

AbsoluteCare is seeking a Community Based Advanced Practice Provider in Washington, DC, to deliver urgent and primary care to members in their community. This role entails working collaboratively with primary care teams to support patients post-hospital discharge, ensuring a smooth transition of care, and engaging in chronic disease management. Qualified candidates will have at least 2 years of experience as a Nurse Practitioner or Physician Assistant, and a strong patient-centered focus.
