Patient Care Coordinator

Patient Care Coordinator Work Location: Core - On Site
Work Hours: 8:00AM-4:30PM
Position Summary This position is responsible for accomplishing various task that contributes to the organization success and patient satisfaction.
Team Environment The team consists of Providers, Medical Assistants, Nurses (RN/LPN), PSA, and Echo Techs, averaging 21 members daily. This is a fast-paced, diverse environment focused on collaboration, compliance, patient care, and operational excellence.
Key Responsibilities
- Obtain accurate and updated patient information, including insurance details
- Perform insurance verification on the date of service
- Obtain patient signatures for required documents
- File, fax, and maintain medical records
- Confirm and schedule appointments
- Answer incoming and outgoing telephone calls promptly and courteously
- Follow Payment Security policies and Optum Medical guidelines
- Manage work queues, referrals, and scheduling requests
- Check In and Check Out patients
- Maintain and work the front desk in-basket pool
- Perform referral documentation and patient follow-up services
- Schedule specialist appointments as needed
- Work cohesively with team members to achieve goals
- Maintain continuity of care through scheduling and tracking systems
- Provide effective communication with patients, families, and healthcare professionals
- Ensure compliance with administrative policies, HIPAA, and OSHA requirements
- Scan, retrieve, and assign documents to the appropriate electronic chart
- Crosstrain and assist at other locations as needed
- Perform other duties as assigned
- Healthcare background preferred
- Minimum 2 years of relevant experience
