Speech Pathologist - Home Health (PRN)

Location: Plano, TX Job Id: 1024 # of Openings: 1 Speech Therapist PRN Home Health Must be willing to travel to Dallas, Richardson & Garland areas. The Speech Pathologist-Therapist/Audiologist (SLP) is a qualified professional who directs, supervises, evaluates and provides skilled speech pathology services on an intermittent basis to clients in their homes. These services are performed in accordance with the physician's orders and other duties as deemed necessary. Responsibilities Evaluates the effectiveness of Speech Pathology intervention and updates the client care plan as needed. Participates in the development of periodic revision of the physician's plan of treatment keeping him/her informed of the patient's status and obtaining additional orders if necessary. Writes clinical/progress notes on the date of the visit and incorporates same in the client's records within 24-48 hours. Communicates with other team members and when appropriate, instructs them in Speech Pathology techniques that they may use while working with the client. Assumes responsibility for self-development by continually striving to improve his/her health care knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organization meetings and individual research and reading. Communicates effectively with all those providing care. Participates with staff, client and physician in discharge activities and completes the Speech Therapy discharge summary within the designated framework of the Agency policies. Confirms, on a weekly basis, the scheduling of visits with the Intake Coordinator to coordinate necessary visits with other personnel. Problem identification and goal setting. Treatment planning in consultation with the physician to achieve goals. Interdisciplinary coordination. Client/family education as appropriate. Provision of equipment required to increase the client's function and independence. Timely recording of all evaluation data, treatments and client's response to therapy interventions. Assuring appropriateness of physician's orders and discussing necessary changes. Participates in case conferences, therapy staff meetings, agency activities and in-service programs as indicated. Supervisory Responsibilities Supervises the Home Health Aide every 2 weeks when speech therapy is the only professional discipline involved in the client's care. Supervises the Physical Therapy Assistant as required by state regulations at least every 2 weeks as outlined in the physical therapy assistant job description. Educational Requirements and Experience Certificate of Clinical Competence (C.C.C.) awarded by the American Speech Language and Hearing Association. Current license in the state of Texas. Current CPR certification. Master’s Degree in Speech-Language Pathology with a Bachelor’s Degree in the field of Speech and Hearing Therapy preferred At least 1 year of supervised work experience At least 1 year of Home Health experience #J-18808-Ljbffr
