Medical Social Worker

Role Overview The Medical Social Worker applies social work expertise and clinical knowledge to educate patients with kidney disease on treatment, health, and wellness options to improve their quality of life. They support patients' social and emotional well-being by conducting psychosocial assessments, participating in care planning, and managing services to help them cope with chronic kidney disease and dialysis treatments. Role Overview The Medical Social Worker applies social work expertise and clinical knowledge to educate patients with kidney disease on treatment, health, and wellness options to improve their quality of life. They support patients' social and emotional well-being by conducting psychosocial assessments, participating in care planning, and managing services to help them cope with chronic kidney disease and dialysis treatments. Key Responsibilities Support patients' emotional and social needs through assessments and care planning. Participate in multidisciplinary team reviews to address psychosocial issues. Educate patients on treatment choices and wellness strategies. Contribute to creating a compassionate, patient-centered care environment. Qualifications & Skills Master's Degree in Social Work from an accredited institution. MSW required; LCSW/ACSW preferred (LCSW in CA). Licensure or certification as per state requirements (e.g., LMSW, LCSW). Preferable experience in health, human services, or healthcare settings. Strong interpersonal skills, goal-oriented, and committed to improving patient outcomes.
