The ideal Speech Language Pathologist (SLP) candidate will have the skills and passion to provide high-quality speech therapy services to our patients, while collaborating with our multidisciplinary team to provide comprehensive care. We are seeking a Speech Language Pathologist (SLP) to join our team on a PRN/Per Diem basis. The ideal Speech Language Pathologist (SLP) candidate will have the skills and passion to provide high-quality speech therapy services to our patients, while collaborating with our multidisciplinary team to provide comprehensive care. Speech Language Pathologist (SLP) Responsibilities
- Assess referred patients that require rehabilitation services
- Provide direct patient care to assess their medical condition, functional capabilities, limitations, and restrictions and potential for rehabilitation
- Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician
- Set realistic and achievable goals for patients
- Document and record patients' conditions
- Educate patients and families in an appropriate therapy method
- Evaluate patients, complete weekly progress notes, recertification, and discharge documentations
- Collaborate with all disciplines to provide best patient-centered treatment and care
- Perform all other duties assigned by the Director of Rehabilitation
- Comply with confidentiality, HIPPA, and Federal Residents' Rights
- Must have graduated from an accredited Speech Therapist college or university
- Must have a valid professional license and registration
- Experience with long-term care and/or geriatric care is preferred
