Type: Full and Part Time, and On-Call (PRN) available Shift/Schedule: FT: Day shift (6am-2pm), 4 days on/2 days off/rotating schedule FT: Swing shift (2-10pm), 4 days on/2 days off/rotating schedule FT: NOC shift (10pm-6am), 4 days on/2 days off/rotating schedule PRN: On-Call, All shifts available, Weekend availability needed Location: Avamere at Park Place - 8445 SW Hemlock St., Portland, OR 97223 - Near Tigard, OR

Responsibilities:

• Care for residents and assist in updating each resident's service plan
• Administer medications ordered by the attending physician, under the direction of the DHS and according to current policies, procedures, and practices
• Provide care for residents and assist with updating service plans.
• Respond to resident calls and emergencies promptly and provide first aid assistance
• Assist with daily living activities (i.e., bathing, dressing, and eating)
• Provide support with residents to caregivers as needed
• Document and communicate resident changes and complete all reports in a timely manner
• Maintain confidentiality of all resident care in accordance with HIPAA guidelines

Qualifications:

• Must be at least 18 years old
• High-School diploma or equivalent
• HCA or CNA license preferred
• CMA certification preferred
• Knowledge and experience caring for and interacting with elders
• Must have medication administration experience in assisted living and/or memory care-6 months to 1 year, preferred
• Must have or obtain First Aid & CPR Certification within 30 days of employment
• Must have or obtain a Food Handlers Card within 30 days of employment
