We follow a unit‑based model of care, with a nurse manager overseeing each residential unit. Responsibilities Demonstrates effective skills in the assessment and review of patients’ medical records. Follows each patient’s individualized plan of care, monitors outcomes to treatments, and communicates issues to the patient’s primary nurse. Demonstrates critical thinking skills to determine appropriate nursing interventions and/or the need for collaboration with the physician or other clinicians. Carries out all nursing/physician orders and documents nursing care and patient responses in accordance with hospital requirements. Demonstrates proficiency in medication administration and applies the Six Rights to safeguard against medication errors. Demonstrates proficiency in all fundamental nursing skills. Applies CDC infection control guidelines for standard precautions and transmission‑based precautions and monitors infection control practices within the work environment. Participates as a member of the interdisciplinary team by giving and receiving pertinent information to colleagues. Maintains on‑going and timely communication with families/guardians on all patient‑related issues. Advocates effectively and ensures patient information is properly communicated when patients transition among healthcare professionals, both internally with the MCMD and externally with outside providers. Demonstrates appropriate communication with patients taking into consideration their ages and functional levels. Communicates effectively and follows up with direct care providers regarding patient care needs. As a Licensed Practical Nurse in a special hospital, performance of additional responsibilities not outlined in this job description may be required in an emergency situation. Additional assistance as a result of staff absences or shortages is also required, including additional weekend and/or holiday time. Full‑time employees also receive an $10,000 signing bonus ($5,000 after six months of continuous employment and the remaining $5,000 after twelve months of continuous employment).
