- *Schedule:** Full Time, 4 Days Onsite, 1 Day Remote
What You'll Do Ensure patients referred to case management meet established criteria.
Assess all patients referred for case management to determine needs.
Develop patient-centered case management plans based on assessments.
Evaluate ability and availability of designated caregiver(s) for support.
Coordinate and implement interventions using evidence-based guidelines.
Recommend additional services to primary care providers as needed.
Conduct ongoing assessments of progress against original goals.
Collaborate with healthcare professionals regarding the plan of care.
Monitor and evaluate patient responses to therapeutic interventions.
Facilitate referrals for home health care, long-term care, and hospice.
Requirements **Education:** Not specified
**Experience:** Case management experience required
Prior nursing home services or geriatrics experience preferred
**Licenses & Certifications:** Current RN License in Washington
