Licensed Practical Nurse - LTC

LPN
We are seeking qualified Licensed Practical Nurses (LPNs) to join our team in a long-term care/specialty nursing facility. This position requires a commitment to delivering high-quality patient care and supporting a diverse range of diagnoses.
Responsibilities
- Provide nursing care to patients according to established standards and practices.
- Assist in the administration of medications and treatments as directed.
- Monitor patient conditions and report changes to the healthcare team.
- Maintain accurate patient records in the electronic medical record system.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
Shift Information
- Shift Length: 12 hours
- Schedule: Three shifts per week, including every other weekend and holiday
- Shift Timing: 0600-1830
Qualifications
- Minimum of 1 year of experience as a Licensed Practical Nurse.
- Current BLS Certification required.
- Experience with electronic medical records (EMR), preferably PCC.
- IV Certification for LPNs is preferred.
Working Environment
This position is located in a facility with 128 beds, where the nurse-to-patient ratio is 1:32. You will be part of a dedicated team providing care for patients with various diagnoses.
Additional Information
- Orientation will be conducted with assigned staff.
- Every third weekend and holiday coverage is required.
