In this role, you will evaluate and treat residents, develop individualized treatment plans, and collaborate with an interdisciplinary team to help residents achieve their highest level of mobility, function, and independence.Key ResponsibilitiesEvaluate residents and develop individualized treatment plans based on physician orders and clinical needsProvide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independenceMonitor resident progress and modify treatment plans as appropriateCommunicate with physicians, nursing staff, residents, families, and interdisciplinary team members regarding treatment goals and outcomesComplete accurate and timely documentation in accordance with facility, state, federal, and payer requirementsParticipate in care planning meetings, rehabilitation conferences, and discharge planning activitiesEducate residents, families, and caregivers on therapy goals, safety techniques, and home exercise programsConduct in-service education and training programs for facility staff as neededEnsure compliance with all state practice acts, facility policies, and regulatory requirementsPromote resident safety, independence, and quality of lifeSupervisory ResponsibilitiesSupervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state regulations and facility policiesQualificationsEducation & ExperienceBachelor's degree in Physical Therapy requiredMaster's or Doctorate degree in Physical Therapy preferredExperience in skilled nursing, rehabilitation, or long-term care preferredExperience with PointClickCare (PCC) and Casamba preferredStrong communication, documentation, and clinical skillsLicenses & CertificationsCurrent Physical Therapist license in South Carolina in good standingMust maintain all required continuing education requirementsMust remain in good standing with the applicable State BoardWhat Makes You a Great FitPassion for helping residents improve mobility and independenceStrong clinical judgment and problem-solving abilitiesExcellent communication and interpersonal skillsDependable and flexibleTeam-oriented mindset with a resident-centered approachCommitment to delivering exceptional care and outcomesWe are an equal opportunity employer.
