Pharmacy Specialist - Ambulatory Navigation Retail Central Fill/Mail

Summary Provides direct patient medication-related care services and comprehensive pharmacy navigation to improve patient outcomes and support value-based care delivery. Serves as a clinical pharmacist fully integrated into daily ambulatory clinic operations and co-located with the care team. Focuses on medication therapy management, medication access and coverage navigation, prior authorization coordination, disease management, population health activities, and patient adherence support. Makes medication-related decisions on drug therapy regarding selection, dosages, routes, methods of administration, medication therapy monitoring, and provision of medication-related information and counseling to individual patients in assigned clinical areas. Works with physicians and other healthcare professionals, with CPA, to optimize medication use through chart reviews, evidence-based recommendations, quality monitoring, and improvement initiatives that support CMS Star Ratings and value-based payment performance. Consults with the patient and physician(s) to identify, resolve and prevent medication related problems, as well as establish mutually agreed upon treatment goals. Provides training/educational support for the development/enhancement of clinical skills in professional staff, pharmacy students, and pharmacy residents. Qualifications Required: Bachelor of Science degree in Pharmacy from a State of Ohio Board of Pharmacy approved institution. Possesses a current Pharmacist License from the State of Ohio Board of Pharmacy. Possesses a current Pharmacy-Based Immunization Delivery certificate from the American Pharmacist Association (APhA), or an equivalent certification from a program recognized by the State of Ohio Board of Pharmacy. Strong knowledge of billing practices for Medication Therapy Management (MTM). Excellent verbal and written communication skills, with ability to educate and communicate with patients, providers, nursing staff, and payers at all levels. Possess a current Basic Life Support (BLS) certification from the American Heart Association (AHA) or obtains within 90 days of hire. Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds. Preferred: Doctor of Pharmacy degree (PharmD) from an ACPE-accredited College of Pharmacy. Completion of an ASHP-accredited PGY1 or PGY2 residency in ambulatory care, community pharmacy, or another relevant specialty. Board Certification in Ambulatory Care Pharmacy, Medication Therapy Management, or another applicable specialty (BPS). Familiarity with MetroHealth EHR workflows, ambulatory care clinic operations, and integrated pharmacy services. Experience with quality improvement methodologies and projects related to HEDIS measures, CMS Star Ratings, or other value-based metrics. Strong problem-solving and critical thinking skills, with ability to work independently and as part of an interdisciplinary team. Knowledge of and ability to navigate pharmacy benefits, formulary restrictions, step-therapy requirements, and tier exceptions. Background in specialty medications, injectable therapies, chronic disease management, or population health. Current immunization and certification including long-acting injectables (APhA, state-approved program). Demonstrated experience with medication therapy management (MTM), prior authorization processes, patient assistance programs, and payer quality programs (e.g., HEDIS, CMS Star Ratings, SUPD, SPC). Experience administering long-acting injectable antipsychotics, biologics, or other specialty injectable medications. Experience working under collaborative practice agreements with ordering privileges for labs and medication adjustments. Physical Requirements May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. May need to lift up to twenty-five (25) pounds on occasion. Ability to communicate in face-to-face, phone, email, and other communications. Ability to see computer monitor and departmental documents.
