Registered Nurse (RN) | Hospital at Home

Location: Avera McKennan Hospital. Work Type: Regular. Shift: Day/Evening/Weekend/Holiday. Pay Range: $32.50 - $48.50 per hour. Sign-on bonus: Up to $15,000. Overview The Registered Nurse – Hospital at Home provides acute-level nursing care to patients receiving hospital services in their homes. The RN delivers comprehensive, evidence-based clinical care consistent with inpatient standards while promoting patient safety, comfort, and recovery in the home setting. The nurse works as part of an interdisciplinary team and is responsible for assessment, clinical intervention, care coordination, and patient education within the Hospital at Home model. Responsibilities Review physician prescriptions and communicate the nursing care plan and pertinent information to other health care team members. Coordinate with other healthcare providers and provide physical, emotional, psychological, and spiritual support to patients, families, and friends. Confer with physicians and provide interventional recommendations based on comprehensive patient assessments. Develop nursing care plans: obtain medical histories, examine patients, document physical condition and psychosocial status, perform labs, provide respiratory care, and establish nursing diagnoses and interventions. Teach healthcare requirements: instruct patients and families on condition, medications, and self-care skills; answer questions and provide resources. Evaluate patient care by adhering to therapeutic standards, measuring health outcomes, reviewing records, and modifying care plans as needed. Maintain a safe and secure environment by following infection‑control policies, medication administration protocols, and controlled substance regulations. Chart patient care services in patient and department records. Keep patient care equipment and supplies in working order, monitor stock levels, and place orders as necessary. Maintain competence in nursing capabilities as outlined by leadership. Qualifications Registered Nurse (RN) with an active state board of nursing license. Minimum 1‑3 years of clinical experience in Med‑Surg, ER, or home health. Ability to work the specified hours and perform required job functions, including visual acuity and effective communication.
