Deliver diabetes self-management education, collaborate with the care team, and coordinate follow‑up plans and referrals. Perform triage and risk assessment using established protocols, with emphasis on Emergency Severity Index (ESI) knowledge. Document care in the electronic health record and communicate care plans across the team; ensure patient safety and adherence to clinical guidelines. Educate patients on diabetes management, lifestyle modifications, medication adherence, and preventive care; support care transitions and care coordination. Requirements Active Registered Nurse license; BLS and ACLS certified; current Certified Diabetes Educator (CDE) certification obtained within the last 3 years prior to start date Clinic experience mandatory, with preference for Diabetes Clinic and Primary Care Clinic exposure Minimum of 3 years of recent RN experience in the applicable specialty Ability to work in a clinic setting; strong critical thinking and triage skills Knowledge of ESI triage and the ability to apply it in clinical scenarios Orientation: Every other Monday during onboarding Benefits and Support Housing assistance available (shared housing, based on availability; no family or pets) Orientation support: Every other Monday onboarding sessions #J-18808-Ljbffr Healthforce
