Description Western Wisconsin Health is looking for a full-time Clinic Social Worker (0.8 FTE) to join the Community Health Improvement department. The Clinic Social Worker works with patients to coordinate community program and clinical service needs. The Social Worker works closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes. All of this is done to support the organization's mission while living our values.
ESSENTIAL DUTIES AND RESPONSIBILITIES
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provides a wide range of social work modalities with individuals, families and groups based upon a social work assessment;
- Conducts on-going assessments, including gathering comprehensive information, to use in developing psychosocial assessments; interventions, and treatment strategies;
- Assists patients in accessing health related services; including but not limited to clinical and wellness services, community based services, and overcoming barriers to obtaining needed medical care and social services;
- Facilitates communication and coordinates services between providers;
- Assists patients in utilizing resources including scheduling appointments and assisting with completion of applications for programs for which they may be eligible;
- Provides advanced care planning services;
- Establishes a positive and supportive relationship with patients and provides feedback;
- Effectively works with people (staff, clients, medical providers, agencies, etc) from diverse backgrounds in reducing cultural and socio-economic barriers between patients and institutions;
- Tracks social determinants of health screening information, data collection, and connection to appropriate community resources;
- Continuously expands knowledge and understanding of community resources, services, and programs provided; human relations and the procedures used in dealing with the public as part of a service or program and participates in relevant community groups as a representative of WWH
- Helps patients develop health management plans and goals;
- Follows up with health management/care plans with both patients and providers;
- Assists patients in understanding care plans and instructions;
- Works collaboratively and effectively within a team;
- Demonstrates awareness, understanding and respect of cultural diversity and patients of all ages and socioeconomic groups; and,
- Ability to track and interpret social determinants of health data and grant program metrics;
- Participate in relevant community groups as a representative of the organization;
- Other duties as assigned.
- Bachelor's degree in social work or related field with CSW
- 1+ years healthcare, community agency, or case management related experience
- CSW
- 2+ years of social service experience in case management or clinic setting
- 0.8 FTE - 64 hours per biweekly pay period
