Occupational Therapist, Acute Experience: 1 year Licenses: OT-OR(Copy needed) Certifications: BLS-AHA(Copy needed); OTR-National Board for Certification in Occupational Therapy(Copy needed) OR Occupational Therapist, Outpatient SSN Required DOB Required Work History: Should not be more than 90 days of gap in last 1 year
