SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY

Serve on Student Support Teams of identified students Support Student Support Team plans to implement interventions that address the root cause of attendance, behavioral, and/or academic challenge(s) with identified students. Provide support internally and externally to schools in the successful implementation of the SST plan for student success. Manage progress monitoring data to provide SST guidance. Collect quantitative and qualitative data from school and community state holders Maintain a log to capture data to inform the SST team of the effectiveness of plan Connect with families as appropriate to support student

CLIENT'S REQUIRED EXPERIENCE & EDUCATION

Minimum of Master's Degree in Social Work Licensed in the State of practice required in Social Work (LMSW/LCSW-C) Medical Social Work experience Ability to undergo and satisfactorily complete given background checks
