Job Summary:
Physical Therapist Outpatient (OP) position located in the region. This outpatient therapy role involves evaluating patients, developing and implementing individualized treatment plans, and promoting patient safety and functional recovery. The position offers an opportunity to work in a dynamic outpatient setting with a focus on community reintegration and patient education.

Job Details:

• Schedule: 8-hour shifts, minimum 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an Accredited School; 6 months to 1+ year of outpatient clinical experience; CPR Certification.
• Preferred Qualifications: 6-month duration of experience highly desired.
• Additional Information: Evaluates all assigned patients and develops, implements, and modifies treatment plans based on patient needs, goals, and physician directions. Responsible for documentation in compliance with state, federal, and intermediary guidelines, including goal setting, functional assessments, and medical necessity letters. Participates in student programs, providing supervision and evaluation. Maintains professional licensure in good standing and pursues ongoing professional development. Adheres to the organizations performance improvement initiatives and ensures ethical practice in accordance with professional codes of conduct. Demonstrates behaviors aligned with the organization's Mission Statement and Core Values, emphasizing safety, teamwork, and quality care.
