Are you a compassionate, skilled Licensed Practical Nurse (LPN) who thrives on independence and meaningful patient relationships? Join our Delta Health, Home Health team and provide high-quality, patient-centered nursing care where it matters most— in the comfort of patients’ homes .

Under the guidance of a Registered Nurse and physician orders, you’ll deliver skilled nursing services that support healing, dignity, and independence.

As a Home Health LPN , you will administer skilled nursing care on an intermittent basis in patients’ residences, following individualized plans of care and agency policies. You’ll play a vital role in patient education, monitoring, and coordination of care across the healthcare team.

• Deliver skilled nursing care per physician orders and care plans
• Implement and evaluate nursing interventions in collaboration with RN
• Monitor patient conditions and report changes promptly
• Administer treatments, medications, and preventive/rehabilitative procedures
• Educate and support patients and families on care needs and self-care
• Maintain accurate, timely clinical documentation
• Participate in care conferences, audits, staff meetings, and in-services
• Assist physicians and RNs with specialized procedures
• Prepare equipment and maintain aseptic technique
• Provide on-call coverage (nights, weekends, and holidays as assigned)

• Graduate of a state-approved Practical/Vocational Nursing program
• Active LPN license in the state of Colorado
• IV Certification upon hire or within 6 months
• Minimum 1 year nursing experience (preferred)
• BLS Certification required upon hire
• Commitment to the mission and values of the Agency

• Confident, independent practice in home settings
• Strong clinical judgment and crisis response skills
• Compassionate, patient-centered approach—especially with elderly and chronically ill patients
• Awareness of family dynamics affected by illness or disability
• Non-judgmental, culturally sensitive mindset
• Ability to care for patients of all ages
• Excellent organization, communication, and time-management skills
• Knowledge of community resources and interdisciplinary collaboration

• Valid driver’s license and reliable personal vehicle
• Current auto insurance and proof maintained on file
• Ability to travel in varying weather conditions and times
• Operational phone or equivalent means of contact

• Ability to engage in physical activity for extended periods
• Lifting up to 50 pounds
• Frequent standing, bending, kneeling, stooping, and squatting
• Normal or correctable vision and hearing
