Registered Nurse - IMC/PCU/Stepdown

Job Summary:

Provides specialized case management nursing services within the IMC/PCU/Stepdown unit, supporting patient transitions, discharge planning, and care coordination to ensure optimal patient outcomes.

Responsibilities:

• 
  Collaborate with the nursing team and healthcare providers to develop comprehensive care plans.
  
  
• 
  Assess patient needs and eligibility for appropriate discharge and home health services.
  
  
• 
  Communicate effectively with patients and families to promote understanding of care plans.
  
  
• 
  Maintain accurate documentation of patient interactions and care activities in CERNER, NATE, and ENSOCARE systems.
  
  
• 
  Support unit operations by participating in self-scheduling and floating to other units as needed.
  
  
• 
  Uphold hospital policies and contribute to a safe, effective, patient-centered environment.
  
  

Qualifications:

• 
  Current registered nurse (RN) license.
  
  
• 
  Minimum of 2 years of experience in IMC, PCU, or stepdown units.
  
  
• 
  Demonstrated ability to work collaboratively within a multidisciplinary team.
  
  
• 
  Strong communication and documentation skills.
  
  
• 
  Ability to adapt to shifting priorities and multitask efficiently.
  
  
• 
  Effective at building trust with patients, families, and team members.
