Licensed Practical Nurse (LPN) Responsibilities

• Provide direct nursing care under the supervision of a Registered Nurse or physician.
• Administer medications and treatments as prescribed.
• Monitor and document vital signs, health status, and changes in patient condition.
• Assist with developing and implementing individualized care plans.
• Maintain accurate and timely patient documentation.
• Provide health education and support to individuals and direct care staff.
• Assist individuals with medical needs while promoting dignity, independence, and quality of life.
• Ensure compliance with agency policies, New York State regulations, and HIPAA standards.
• Respond appropriately to medical emergencies and communicate effectively with the interdisciplinary healthcare team.
• Maintain a safe, clean, and therapeutic environment.

Qualifications

• Current, unrestricted New York State Licensed Practical Nurse (LPN) license.
• Minimum of one (1) year of LPN experience preferred.
• Experience working with individuals with intellectual and developmental disabilities is a plus, but not required.
• Current BLS/CPR Certification .
• Excellent communication, organizational, and critical thinking skills.
• Ability to work independently and as part of a multidisciplinary team.
• Reliable transportation.
