Schedule: 4x10-hour shifts (40 hours/week) Shift: Day Shift, 8:00 AM – 6:30 PM Call: None Experience Required: Minimum 1 year acute care experience (Required) Position Summary & Responsibilities Coordinate patient care across the continuum from admission to discharge Collaborate with interdisciplinary teams to optimize patient outcomes Monitor care plans, resource utilization, and discharge planning Act as liaison between patients, families, providers, and payors Support compliance, documentation, and quality improvement efforts Job Requirements Education: BSN (Preferred) Experience: Minimum 1 year acute care experience (Required) Licensure/Certifications: Active RN license in the state of practice (Required) Case Management Certification (Preferred) Key Duties Coordinate interdisciplinary care and ensure timely, appropriate treatment Review resource utilization, length of stay, and discharge plans with providers Manage complex patient populations and frequent ED utilizers Facilitate transfers and continuity of care across facilities Ensure regulatory compliance and maintain accurate documentation Organizational Requirements Must meet vaccination requirements (medical/religious exemptions may apply) Commitment to patient safety and quality care standards #J-18808-Ljbffr TEEMA Solutions Group
