Description
Provides intermediate-to-advanced human services support to community members, including individuals age 17 1/2 and older with disabilities and older adults. Assists the public and their support systems by providing information, options counseling, long-term care program eligibility determination, enrollment and disenrollment counseling, advocacy, and connections to appropriate community-based services and supports. Work is performed under the limited supervision of the Aging and Disability Resource (ADRC) Supervisor and Aging and Disability Services Manager. All duties are performed within the scope of services of the Aging and Disability Resource Center.
It is unlikely an employee will perform all the duties listed on a regular basis, nor is the list exhaustive in the sense it covers all the duties that an employee may be required to perform. The examples are merely indicative, not restrictive.

Examples of Duties
  1. Provide customer driven information and assistance via phone, email, or in person on a wide variety of local resources that are accurate, objective, and relevant to the individual's expressed goals.
  2. Provide option counseling to assist consumers, their families, and caregivers about the options available to meet long term care needs and factors to consider in making long term decisions.
  3. Assist individuals with the functional and financial requirements to access publicly funded Long Term Care programs.
  4. Assist individuals with the Medicaid application process.
  5. Provide information about enrollment and/or disenrollment options for individuals looking to enroll or disenroll from Long Term Care programs.
  6. Provide short term care coordination to individuals who have complex care needs.
  7. Participate in marketing and outreach activities.
  8. Collaborate with other service providers and community organizations.
  9. Advocate for individuals and groups within the target populations when needed services are not adequately provided. This may include helping individuals navigate barriers or working with partners and systems to address service gaps and improve access or quality.
  10. Performs other related functions and duties as assigned or required and may serve on related work groups, projects, committees, and/or community projects as assigned.

Essential Qualifications
Bachelor's degree in social work, sociology, human services, or related field and moderate experience in State and Federal support programs, social work, administrative operations, human services, or equivalent combination of education and experience. Obtain Adult Long Term Care Functional Screener certification within six (6) months of hire. Valid driver's license in the State of Wisconsin.

Familiar with County and department policies and procedures to include emergency and inclement weather policies; ability to prioritize and manage multiple, concurrent projects; attention to detail and ability to effectively organize time and anticipate, plan and successfully respond to changing circumstances; ability to complete long term care functional screening for individuals wanting services but cannot privately pay; ability to assist and counsel clients; knowledge of state-run long term care programs; ability to make arithmetic computations using whole numbers, fractions and decimals; ability to operate and use standard office equipment; knowledge and ability to use standard office and position-specific software; ability to effectively communicate both verbally and in writing; ability to establish and maintain effective working relationships with co-workers, supervisors, other departments and the general public.
PHYSICAL DEMANDS

This work requires the occasional exertion of up to 10 pounds of force; work regularly requires sitting, speaking or hearing and using hands to finger, handle or feel, frequently requires repetitive motions and occasionally requires standing, walking and lifting; work has standard vision requirements; vocal communication is required for expressing or exchanging ideas by means of the spoken word and conveying detailed or important instructions to others accurately, loudly or quickly; hearing is required to perceive information at normal spoken word levels and to receive detailed information through oral communications and/or to make fine distinctions in sound; work requires preparing and analyzing written or computer data, operating machines and observing general surroundings and activities; work has no exposure to environmental conditions; work is generally in a moderately noisy location (e.g. business office, light traffic).

Exposure to bloodborne pathogens is considered low for this position.

The County offers health insurance coverage through UMR. The County pays 85% of the premium for both single and family coverage, along with a contribution to the Health Savings Account (HSA).

Dental, Vision, and supplemental insurance is also available, with the employee paying the premium.

The County offers a basic life insurance plan (at the County expense) for regular employees. There are also supplemental plans available for the employee to purchase.

The County provides a retirement account in the Wisconsin Retirement System (WRS) for eligible employees. Employees pay one-half of the required contribution; the County pays the other half.

The County provides a confidential Employee Assistance Program (EAP) to all employees and their family members, at no cost to the employee.
