Schedule: 8 hours

Assignment Details:

Contract Weeks: 13
Guaranteed Hours: 40

Requirements:

Current state license as a Registered Nurse
Homecare Homebase Software Experience
OASIS Experience
CHPN certification - Preferred
Proof of current CPR certification
Current driver's license
Must have reliable transportation and insurance
1-2 years experience as a Registered Nurse in a clinical care setting
2+ years Home health experience
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program
Bachelor of Science in Nursing - Preferred

Roles & Responsibilities:

Assess home health and hospice patients to identify their physical, psychosocial, and environmental needs.
Implement, document, and evaluate the Plan of Care to ensure quality and continuity of care.
Educate patients and families regarding care, medication, and safety practices tailored to their needs.
Supervise and provide clinical direction to home health aides to ensure quality service.
Monitor assigned cases for compliance with third-party payer requirements.
Provide effective communication with patients, families, and the healthcare team.

Compliance / Notes:

Performs on-call responsibilities as assigned.
Attends interdisciplinary team meetings when necessary.
