Job Title: Physical Therapist (PT) – SNF / LTC
Location: Hilliard, FL
Assignment Length: 13 Weeks
Schedule: Monday–Friday, Day Shift (8-hour shifts)
Guaranteed Hours: 35 hours/week

Compensation:

• Local Rate: $46/hour
• Travel Rate: $1,946/week

We are seeking an experienced Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) and Long-Term Care (LTC) setting. The role involves providing therapy services to geriatric and older adult populations, including both subacute rehabilitation and long-term care patients.

• Evaluate patients and develop individualized treatment plans
• Deliver physical therapy interventions in SNF and LTC settings
• Assess mobility, strength, balance, and functional abilities
• Collaborate with interdisciplinary healthcare teams
• Educate patients, families, and caregivers on treatment plans and exercises
• Maintain accurate and timely clinical documentation
• Monitor patient progress and adjust treatment plans accordingly

• Active Physical Therapist license in Florida (or ability to obtain prior to start)
• Graduate of an accredited Physical Therapy program
• Experience in Skilled Nursing Facility (SNF), Subacute Rehab, or Long-Term Care
• Experience working with geriatric populations
• Strong communication, documentation, and patient care skills
• Ability to work independently and within a team

• Prior travel or contract experience
• Experience with post-surgical, orthopedic, neurological, or complex rehab cases
• Familiarity with SNF documentation systems and compliance standards

• 13-week contract
• Guaranteed 35 hours per week
• Monday–Friday schedule (no weekends)
• Day shift (8 hours)
