POSITION SUMMARY - RN Case Manager
POSITION DUTIES - Coordinates patient care, case management, and discharge planning by assessing patients' medical, functional, and social needs
MINIMUM REQUIRED QUALIFICATIONS -
- MT/compact RN license
- BLS
- BSN
- 3 years of experience as a Registered Nurse in an acute care setting
- Hospital care management experience
- Knowledge of Medical Terminology
- Knowledge of Desk Top software including Power Point and Excel, computer skills for data entry
- 5 years nursing experience
- ACM or CCM certification
LENGTH OF ASSIGNMENT - 13 weeks
SHIFT / HOURS PER WEEK - 8hr days
SYSTEMS - Epic
START DATE - 11/2/2026
