Physical Therapist (Hamilton/Bitterroot Valley)

Partners In Home Care (PIHC), Western Montana's leading non-profit home health provider since 1987, is seeking a compassionate and skilled Physical Therapist to join our growing Home Health team serving the Bitterroot Valley. This position offers the opportunity to build meaningful relationships with patients while delivering personalized, one-on-one care in the comfort of their homes across Ravalli County, one of Montana's most scenic communities. Department: Home Health Employment Type: Full-Time or Part-Time Schedule: Monday–Friday, with flexibility based on FTE and patient care needs Location: Based out of Ravalli County. May require travel to other counties within our service area. Essential Duties and Responsibilities Conduct in-home physical therapy evaluations and develop individualized plans of care Provide skilled, evidence-based physical therapy treatments as ordered by the physician Adjust treatment frequency and duration based on patient progress and reassessment Complete and score OASIS assessments accurately and in compliance with Medicare regulations Maintain knowledge of Medicare Conditions of Participation and payer requirements Perform therapeutic interventions, including wound care when appropriate Document all care and communication using point-of-care electronic systems Coordinate daily schedules, participate in interdisciplinary care conferences, and support quality improvement initiatives Supervise Physical Therapist Assistants (PTAs) and CNAs as applicable Manage patient care episodes including admissions, re-evaluations, and discharges Maintain active Montana PT licensure, required continuing education, and ethical professional standards Participate in agency training and professional development activities

REQUIREMENTS
