Title: RN / MSW – Care Coordination / Case Management – Contract
Location: Everett, Washington (Level II Trauma Center – Multi Campus Facility)
Duration: 13 Weeks
Payrate: Starting at 45$/hour
Shift: 4x10 Day Shift (Start ASAP)
Position Overview"Navitas Healthcare, LLC" is seeking an experienced RN or MSW for a Case Management / Care Coordination role in an acute care hospital setting. The position involves discharge planning, utilization review support, and coordinating patient care across multiple hospital campuses.
Key Responsibilities- Perform discharge planning and care coordination for acute care patients
- Collaborate with physicians, nurses, and interdisciplinary teams
- Support case management activities across Med-Surg, ED, and long length-of-stay patients
- Assess patient needs and develop safe discharge plans
- Coordinate post-acute care services and community resources
- Ensure timely documentation in EMR (EPIC)
- Participate in care conferences and patient progress reviews
- Assist with utilization and utilization review workflows
- Work across multiple campuses as needed
- Ensure smooth patient transitions and continuity of care
- Active RN License (or MSW eligible for case management role)
- Minimum 1 year of acute care experience required
- Discharge planning / care coordination experience required
- Strong preference for Med-Surg, ED, or LLOS case management experience
- BLS Certification required (for RN roles)
- Ability to work in fast-paced hospital environment
- Strong communication and organizational skills
- EPIC charting experience preferred
