Job Description

The Licensed Vocational Nurse (LVN) is responsible for assessing, planning, and implementing nursing care for residents in a skilled nursing environment under the supervision of a Registered Nurse and Director of Nursing. The LVN will administer medications following the eight medication rights to minimize the risk of errors and ensure continuity of care during shift transitions.

Roles & Responsibilities

• Assess resident conditions and nursing needs, taking appropriate nursing actions.
• Manage and educate residents and families regarding pain management techniques.
• Document changes in residents' conditions and care on progress notes and care plans.
• Conduct waived testing (point-of-care testing) according to policies and procedures.
• Administer medications while adhering to the eight medication rights.
• Notify physicians about changes in residents' conditions and ensure follow-up actions are taken.
• Participate in handoff reporting by providing comprehensive shift reports.
• Implement individualized resident care plans based on interdisciplinary team assessments and goals.
• Ensure residents and their families are treated with respect and dignity, addressing psychosocial and spiritual needs.
• Supervise CNAs and other healthcare team members to assure effective care delivery.
• Act as a resident advocate by prioritizing resident rights and needs.
• Maintain calm during emergencies and stressful situations, providing reassurance to residents and team members.
• Follow infection control procedures and supervise staff adherence to these guidelines.
• Participate in the orientation of new staff and assist in performance evaluations as needed.

Qualifications & Skills

• Graduate of an accredited LVN program.
• Current California Licensed Vocational Nurse.
• Current Basic Life Support (BLS) certification.
• Ability to read and communicate effectively in English; additional languages preferred.
• Basic computer proficiency; experience with Electronic Health Records is preferred.
