Job Summary
The Children’s Long-Term Support (CLTS) Service and Support Coordinator provides support and service coordination and assists participants and their family to achieve an inclusive, interdependent, and self-empowered life. The participant’s support and service coordinator (SSC) needs knowledge of available programs, resources, and services to create an individualized service plan that best supports the participant and family in pursuit of their desired outcomes. This includes connecting families to supports throughout the community and promotes the participant’s self-determination and involvement in all facets of community life. This position focuses on physical, developmental, and social and emotional disabilities. This also consists of working with the family to develop an Individualized Service Plan (ISP) that will include the services and supports that are needed to address the child and family’s unique needs. Service coordinators continue to work with children and their families to assure that all needs are being met over time and under changing circumstances.
The following duties are primarily performed and are essential for this position. Employees are expected to be able to perform each of these job duties satisfactorily and successfully with or without reasonable accommodations to be qualified for the position. Other duties may be required and assigned. 30% Intensive Case Management:
Assesses all aspects of an individual and family (health/physical functioning, AODA, mental health, and financial); implement a comprehensive service plan to meet the individual needs and outcomes in a cost-effective manner and evaluate progress. Coordinates with families and their child to develop a clear, family-centered individual services plan. Triages and manages complex situations, including assisting families in crisis. Finds appropriate resources, providing family direction/support in resource utilization, making referrals to other programs/providers, and connecting with formal and informal supports already in place (i.e., school, doctors/specialists/ABA). Conducting home visits at least every 6 months ( ideally quarterly ) and completing corresponding paperwork. Makes difficult decisions and has conversations that may be complex, including completing a CPS referral or discussion with outside providers about concerns. Stays up to date on DMS, DHS publications, Medicaid rules and regulations, maintain certification to provide case management and to complete functional screens. Completes the Mandated Reporter Online Training. SSC is required to be knowledgeable of the service delivery system, the availability of integrated services and resources, or the need for such services and resources to be developed. Assessing and periodically reassessing the participant’s health, safety, and functional capacity. Identifying vulnerable and high-risk children and ensuring heightened care coordination, collaborating with other agencies when applicable. Assist the consumer and family to access Early and Periodic Screening, Diagnostic and Treatment (EPSDT) benefit (known as HealthCheck), Medicaid State Plan services, CCOP funding, private health insurance, and to monitor and assist with the Medicaid financial annual review. Assists with establishing financial, functional, and all other aspects of Medicaid waiver eligibility. 35% Documentation:
Entering all case documentation into the Human Services Department’s Electronic Health Record per the documentation policy within 72 hours. Manages documentation of 20 or more required forms per case, including but not limited to: Narrative, ISP, Outcomes, exceptional expense notification, high cost, CITR, and service request forms. Monitors and reviews CLTS Provider progress notes to maintain up to date information on participant progress. Collection and reviewing of outside reports from school and private therapies. Completes a child-centered assessment by gathering current, comprehensive information about the child. Strives to use a systems approach to meeting a child’s unique needs, skills, supports, and environment. Works with the child and family to identify and record their desired outcomes. Explores the child’s and family’s preferences for service delivery, living arrangement, medical care, and community participation. Completes the ISP, working collaboratively with the family to determine which supports, services, and environmental modifications will benefit the child, build on the child’s strengths, and maximize the child’s independence and community participation. Sends outcomes to direct service providers and ensure providers sign and return outcomes to SSC. Schedules and authorizes supports and services listed on the ISP. Calculates parental fees annually and as needed. Submit service request forms to demonstrate needs assessment for new services/items. Keeps a detailed Outlook calendar. 15% Billing/Fiscal Responsibilities:
Bills time worked per productivity standard. Understands what services can be covered under another funding source, work with families/providers to obtain necessary service denials/approvals. Documents contact with the family and child in the child’s record. Ensures that CLTS Waiver Program services are funded as the payer of last resort. 5% Assessment and Ongoing Eligibility Determinations:
Reviews a client’s current services and service needs during monthly collateral/direct contacts, six-month reviews, and annual recertification’s in order to determine if services continue to be appropriate or other needs arise. Calculates service rates for clients approved CLTS services following the Rate Setting Initiative. Informs families about denial-of-service requests and sends an NOA in a timely manner. 5% Cross-System Collaboration:
Collaborates and coordinates with other programs the client may be involved with including, but not limited to: Birth to Three, CCS, Juvenile Justice, CPS, and the ADRC. Makes referrals in a timely and appropriate manner. Communicates/collaborates with staff in other departments on shared case needs and updates in order to provide a wraparound approach to client service needs. Works with outside supports on client needs including attending and being a valued team member on a child’s IEP team, communicating with physicians, therapists, etc., for case updates/needs/obtaining orders. Sends referrals to other departments. 5% Provider Collaboration:
Makes a collateral contact to review the child’s and family’s current needs and update supports and services accordingly. Provides authorizations for each CLTS Waiver billable service and ensuring appropriate billing service; educates and provides referral services to the general public and agency staff regarding CLTS. Coordinates authorization from third party administrators for necessary services, collaborates with providers to ensure quality services are provided, and communicates progress with supported documentation. Ensures all CLTS funded services are delivered in accordance with program requirements and service providers meet required qualifications. Guides potential providers on how to/where to submit information to become a certified provider and direct them to where they can receive additional auth support, work with providers to make sure they submit and understand the necessary documentation they need to submit for cases (DLST and Counseling and Therapeutic reports, signed outcomes) as well as documentation needed to become a certified provider. 3% Miscellaneous:
Collaborates with CLTS Team members. Understands CCOP & participate in the quarterly Children Come First Advisory Meetings. Attends unit and division meetings, team meetings, supervision, and case consultations. Active involvement in training new hires. 2% Programmatic and Transactional Activities:
Provides transitional support for CLTS Waiver Program enrollment and disenrollment. Assists the participant to locate safe and appropriate housing, including the determination of the efficacy of substitute care settings. Assists the participant to pursue vocational and/or educational opportunities. Supporting transition-planning processes (for example, child-adult at age 14) in accordance with CLTS transition requirements. Supervision Exercised
None Minimum Education Qualifications Education and/or Experience Requirements:
- Bachelor’s Degree in Human Services accepted, Master’s preferred.
- Certified Children’s Functional Screener Certification may be obtained after hire.
- Certified Children's Long Term Support Waiver (case manager) Certification may be obtained after hire.
- Valid Wisconsin Driver’s License and Insurance required upon hire.

- Ability to maintain accurate, timely, and comprehensive documentation in compliance with state standards as well as agency policy and procedures.

- Requires a valid Wisconsin Driver's License with a good driving record and liability insurance.
- Ability to travel throughout the county sometimes daily at times with personnel vehicle.
