Identifies expected outcomes or goals of care based on a plan to individualize care, and in collaboration with patients, families, groups, communities or populations served. Develops a plan that prescribes strategies to attain expected outcomes or goals. Nurses systematically implement the individualized care plan, facilitates the coordination of care with inter and intra-professional colleagues and employs strategies for health teaching and promotion. Evaluates progress toward achieving defined outcomes/goals and changes the plan if necessary. Implements evidenced-based approaches to care. What You Will Need A Diploma from an accredited school/college of nursing is required - BSN or ADN BLS provider (accredited by the American Heart Association) required prior to hire Current Registered Nurse License State of Illinois 1-2 years of previous experience in a clinical setting. Compensation Salary Range: $31.62 - $50.79 per hour Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
