RN Case Manager: Discharge & Care Coordination

UAB Medicine

UAB Medicine is seeking an RN Case Manager to coordinate comprehensive care from admission to discharge at UAB St Vincent's Birmingham. The role requires active RN licensure in Alabama, a diploma in nursing, and BLS certification within 30 days of hire. You will work Monday–Friday with occasional rotations, in a major health system focused on high-quality patient outcomes. The position offers the chance to impact across the care continuum, collaborate with an interdisciplinary team, and #J-18808-Ljbffr UAB Medicine
