Physical Therapist – Skilled Nursing Facility travel assignment in Lancaster, New Hampshire. Start date 07/13/2026 for 13 weeks. Guaranteed 35 hours per week with a daytime shift from 08:00 to 16:30. Responsibilities
- Conduct comprehensive PT assessments for residents and develop individualized plans of care addressing mobility, strength, balance, gait, and functional tasks.
- Deliver and modify therapeutic interventions per the plan of care to promote functional independence and reduce risk of falls.
- Monitor resident progress, document evaluations, treatment notes, and outcomes in the medical record; adjust plan of care as needed.
- Collaborate with physicians, nursing staff, therapy teammates, and families to coordinate care and support discharge planning.
- Educate residents and families on exercises, safety, and home programs; ensure safety and infection control during all therapy sessions.

Requirements
- Ability to work in a skilled nursing facility setting.
- Availability for a 13-week travel assignment with a guaranteed 35 hours per week.
- Ability to deliver therapy in a daytime schedule 08:00–16:30.
- Strong communication and teamwork skills for interdisciplinary care.
- Comfortable with documentation and credentialing processes and maintaining accurate records.
