Client Development and Intake Services Coordinator

HARMONY HEALTH HOME CARE, INC.

The Client Development and Intake Services Coordinator is responsible for managing the client intake and onboarding process, supporting waiver applications, coordinating with state and local agencies, and driving client growth through outreach and referral relationships. This role also oversees day-to-day operations of Harmony’s Philadelphia satellite office and supports ongoing client renewals and compliance. Key Responsibilities Manage the full client intake process from recruitment through onboarding. Coordinate with County Assistance Offices, Maximus, and PCA/AAA to advance applications. Guide clients through the waiver process, including financial documentation. Assist clients with banking tasks, online account setup, and collection of required records (5-year bank statements, Social Security award letter, or “no bank” letter). Collect and maintain client demographic and contact information. Schedule and confirm Maximus and PCA assessments and provide reminders to clients. Ensure timely completion of physician certification forms. Lead marketing and outreach initiatives to bring on new clients. Develop and maintain referral relationships with hospitals, physicians, rehabilitation centers, senior facilities, and community organizations. Recruit caregivers through advertising and partnership outreach. Support renewals and recertifications for existing clients. Submit monthly reports on outreach efforts, referrals, and intake volumes. Oversee day-to-day operations of the Philadelphia satellite office, ensuring professional operation, staff coordination, and adherence to Harmony’s administrative and compliance standards. Training and Work Location Initial two-week training will take place at the main office located at: 65 Buck Rd, Unit B, Huntingdon Valley, PA 19006 After training, the primary work location will be: 2061 N 62nd St, Philadelphia, PA 19151 Qualifications Experience in client intake, outreach, healthcare coordination, or related fields preferred. Strong communication skills and ability to work directly with clients and families. High attention to detail and strong organizational skills. Comfortable working with government agencies and community partners. Ability to work independently while following structured processes. Compensation and Performance Incentives Compensation is competitive and based on experience, qualifications, and performance.
