Therapist - Speech Language Pathologist

Job Summary:
The Speech Language Pathologist provides specialized assessment and therapy services to residents with communication and swallowing disorders, promoting independence and optimal function. They collaborate with healthcare teams to develop and implement individualized care plans in a long-term care setting.

Responsibilities:

• Evaluate residents and establish comprehensive therapy plans, including goal setting and treatment frequency.
• Perform evaluative and therapeutic procedures for communication and dysphagia, including modified barium swallow assessments.
• Monitor and assess treatment progress, modifying plans as needed to ensure optimal outcomes.
• Document all clinical interactions accurately and accessibly, ensuring compliance with departmental standards.
• Educate residents and caregivers on communication and swallowing strategies, including assistive device use.
• Collaborate with physicians and interdisciplinary teams to coordinate resident care and discharge planning.

Qualifications:

• Hold a master’s degree in speech-language pathology and a valid SLP license.
• Possess a Certificate of Clinical Competence.
• Demonstrate strong communication, interpersonal, and documentation skills.
• Be proficient in Microsoft Office and familiar with healthcare documentation standards.
• Have prior experience in long-term care or skilled nursing facilities (preferred).
• Maintain current BLS certification (preferred).
