Job Title - RN Care Manager Department - PACE Reports to - Clinical Services Director Work Location: 1214 Vaughn Rd, Burlington, NC 27217 (Piedmont Health SeniorCare in Burlington) Schedule: Monday through Friday, 8:00am - 5:00pm (Please note that this role also requires after hours availability on a rotational basis for urgent home visits between the hours of 8:00am -5:00pm on the weekends) Benefits - Medical, Dental, Vision, Life Insurance (Short & Long Term Disability) 403(b) Plan Paid Holidays CME (Continuing Medical Education) Job Summary: Working under the direct supervision of the Clinical Services Director, the primary responsibility of the RN Care Manager is to provide direct and indirect nursing care to assigned patients by utilizing critical thinking skills to assist in the management of chronic diseases, clinical problem solving and decision making in collaboration with the IDT. The RN Care Manager works independently in the application of advanced nursing knowledge and skills relative to NCBON scope of practice, competency and experience. Participates in call rotation, performs home visits, end of life/palliative (aka Pathways), and nursing care per nursing protocol. Completes professional performance requirements annually. Education/Experience: Graduate of an accredited school of nursing. Preferred: Has practiced nursing in the last 3 years. Prefer experience in home care or long term care setting, and ideally, PACE experience. Two years of acute care management experience is preferred and/or 1 year of community based nursing experience. Must possess knowledge and skills necessary to treat patients Meet a standardized set of competencies for the specific position established by PHSC and approved by CMS before working independently Excellent interpersonal, oral and written communication skills Licensure, Registry or Certification Required: RN currently licensed in NC; present credentials which demonstrate ability to meet performance criteria for the RN position; Valid NC driver’s license. Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment. Pay Range: $71,088 - $95,536 (commensurate with years of experience)
