Position Overview

This position involves coordinating and directing the provision of home nursing care according to physician's orders and agency policies. The successful candidate will apply their nursing knowledge through competent nursing processes while supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.

Location

Coverage includes the following areas: Gloucester, Manchester by the Sea, Rowley, Ipswich, Essex, Topsfield, Hamilton, Wenham, Beverly, Danvers, Peabody, Lynnfield, Lynn, and Saugus in Massachusetts.

Eligibility Requirements

• Experience: Minimum of 1 full year of experience in Adult Home Health or Hospice care.
• Certifications: Current CPR and BLS certifications required.
• Licensure: Must have a valid license in hand prior to starting.
• Travel: Candidates must be willing to travel up to 75 miles from their assigned care center.

Position Details

• Shift: Day shift, 5 days per week, from 8:00 AM to 5:00 PM.
• Average Patient Load: 5-10 patients per day, translating to approximately 30 productivity points per week.
• Travel Requirements: Home visits are expected, and reliable transportation is necessary. Mileage will be compensated at the IRS standard rate.
• Documentation: Proficiency in HomeCare HomeBase (HCHB) is essential, with recent use within the last 6 months preferred.
• Contract Duration: The assignment is for a period of 13 weeks.

Additional Information

• All documentation must be completed within 24 hours of each patient appointment.
• There are no guaranteed hours; productivity expectations align with hitting a minimum of 30 points per week.
• Regular communication or meetings at the office may be required.
• Local candidates are accepted at the same rate as travelers.
