With advanced training and expertise, the APP focuses on subspecialized care, including chronic illness and disease management, ensuring continuity of care throughout the patient’s healthcare journey, which may span multiple years. This role involves specialization in a specific organ system, where the APP provides expert consultation within their area of expertise as appropriate. The APP is integral to supporting patients across the continuum of care in both inpatient and outpatient specialty settings, collaborating with multidisciplinary teams to achieve optimal patient outcomes..Outpatient PositionPosition Hours:4 - 10 Hour Shifts Per Week (8:00am - 6:00pm)WeekdaysNo Weekends or HolidaysDepartment Summary:Welcome to the VPEC clinic, the clinic is located at Vanderbilt Health at One Hundred Oaks, 719 Thompson Lane Suite 21100 Nashville, TN 37204. We provide preoperative anesthetic evaluations for individuals undergoing elective surgery for the adult population. We are looking for highly motivated individuals that would like to join a growing clinic. Our excellent, patient centered care is directed and/or delivered by a collaborative team made up of Attending Anesthesiologist’s, Resident Physicians and Nurse Practitioners. The Leadership Team is comprised of a Medical Director, Administrative Director, Clinic Manager, a lead Clinical NP. Clinic staff members include Registered Nurses, Licensed Practical Nurses and Patient Services Representatives. Various support resources are also available, such as, social work, referral specialists, laboratory and radiology. We are committed to providing excellent service to our patients and a supportive and caring work environment for our staff.Nurse Practitioner (FNP, ANP, AGPCNP, AGACNP) or Physician Assistant (PA)1 or more years of APP experience requiredKEY RESPONSIBILITIESObtains and documents a health history; performs and documents complete, system-focused, or symptom-specific physical examination, assessment and plan of care.In collaboration, provides healthcare services for primary, acute and complex care of patients and manages patients’ overall care, identifying expected outcomes for diagnoses.Orders, performs, interprets and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.Forms differential diagnoses and treats acute and chronic conditions.Prescribes therapeutic interventions for both pharmacologic, non-pharmacologic and surgical, needed to achieve expected outcomes.Utilizes evidence-based, approved practice protocols in planning and implementing care.Initiates appropriate referrals and consultationsProvides specialty specific consultation services upon request and within a specialty scope of practice.Facilitates the patient’s transition between and within health care settings.The responsibilities listed are a general overview of the position and additional duties may be assigned.TECHNICAL CAPABILITIESHealth Systems and Policy Management (Advanced): Demonstrates sufficient fundamental proficiency in behaviors that reflect a commitment to continuous professional development, ethical practice, an understanding and a responsible attitude toward their patients, their profession and society.
