Casey Rehab & Nursing is hiring a full-time Social Services Director for our skilled nursing facility in Casey, IL.
Responsibilities include:
- Managing admissions and discharge planning
- Completing social services assessments (BIMS, PHQ-9, trauma screening)
- Coordinating care plan meetings
- Supporting residents and families with concerns and transition planning
- Maintaining documentation in PointClickCare
- Assisting with Medicaid-related processes and advanced directives
Qualifications:
- Experience in social work, admissions, or discharge planning in long-term care required
- Knowledge of Medicaid process required
- Degree in social services or related field preferred
