Care Manager

Overview We are seeking a dedicated and compassionate Care Manager to join our team. As a Care Manager, you will be responsible for overseeing and coordinating the care of patients, ensuring their well-being and providing support to both patients and their families. Responsibilities Assess patient needs and develop individualized care plans Coordinate and schedule medical appointments and procedures Monitor patient progress and provide ongoing support Collaborate with healthcare professionals to ensure comprehensive care Educate patients and families on proper self-care techniques Maintain accurate medical documentation and records Provide assistance with activities of daily living as needed Implement dementia care strategies for patients with memory impairments Utilize electronic medical record systems such as Cerner, eClinicalWorks, etc. Administer IV infusions and perform spinal taps as required Qualifications Bachelor's degree in Nursing or related field (preferred) Previous experience in primary care or medical office setting Strong knowledge of physiology and medical terminology Familiarity with dementia care techniques and memory care practices Proficiency in using electronic medical record systems (Cerner, eClinicalWorks, etc.) Excellent communication and interpersonal skills Ability to work effectively in a team environment Compassionate and empathetic nature Benefits We offer competitive pay rates and comprehensive benefits packages for our employees.
